Yes, that's fine. Just do your best when you write down the names, addresses, and amounts owed to your creditors. If possible, please Google around to find info you don't know off the top of your head.

Medical bills are a major cause of bankruptcy filings in the U.S. If you are filing bankruptcy to deal with medical and other debts, you'll be required to fill out paperwork. It will ask for the names, addresses, and amounts owed to your creditors.
If you don't remember your doctor's names that's okay. But if possible, please do a search to find the information you don't know off the top of your head. It's very important that the doctor's office receive a notice of your bankruptcy filing to ensure the debt will be eliminated.
